Description JOB SUMMARY Under the direction of the Disease Management (DM)/High Risk Case Manager and Ambulatory Case Management (ACM) leadership, the Disease Management High Risk Coordinator supports the identification, outreach, monitoring, and follow-up of Disease Management (DM) and High-Risk members enrolled in the ACM Program. The role facilitates care coordination activities, patient engagement, data tracking, and communication with providers and interdisciplinary teams in alignment with health plan and organizational guidelines. Requirements MINIMUM & PREFERRED QUALIFICATIONS Education/Training Minimum: High school diploma or equivalent Preferred: Associate’s degree Experience Minimum: 2+ years of care management or healthcare coordination experience. 1+ years of utilization review experience. Preferred: Experience supporting disease management or high-risk populations. Any combination of educational and work experience that would be equivalent to the stated minimum requirements would qualify for consideration of this position. Skills, Knowledge & Abilities · Proficiency in Microsoft Office applications (Word, Excel, Outlook) · Strong verbal and written communication skills · Ability to engage and communicate effectively with diverse patient populations · Working knowledge of care coordination and utilization review processes · Strong organizational skills and attention to detail · Ability to prioritize tasks and manage multiple assignments · Ability to work collaboratively within an interdisciplinary healthcare team · Understanding of confidentiality and HIPAA requirements PHYSICAL, MENTAL & ENVIRONMENTAL REQUIREMENTS The physical, mental, and environmental demands described here are representative of those required to successfully perform the essential functions of this job.
