Description JOB SUMMARY Under the direction of the Manager of Ambulatory Case Management, the LVN Supervisor, Disease Case Management, supports and coordinates care for high-risk patients enrolled in disease and case management programs. This role assists in ensuring patients with chronic or complex medical conditions receive timely, continuous, and well-coordinated care. The LVN Supervisor performs clinical and administrative disease-management functions within LVN scope of practice, including patient outreach, monitoring, education reinforcement, care coordination, and communication between providers and interdisciplinary teams. The position also provides workflow guidance to case management support staff, assists with program data tracking, and supports quality improvement efforts. This is a working supervisor role in which the LVN performs direct coordination duties while overseeing daily operational functions of the disease management support team. Requirements MINIMUM & PREFERRED QUALIFICATIONS Education/Training Minimum: Graduate of an accredited License Vocational Nurse program. Preferred: Additional training or coursework in chronic disease management, ambulatory care, or care coordination. Experience Minimum: 2+ years of case management experience within a managed care or healthcare facility. Preferred: Prior supervisory or lead experience in care coordination or clinical operations. Any combination of educational and work experience that would be equivalent to the stated minimum requirements would qualify for consideration of this position. Certification(s) Current State LVN License Current BLS Certification Skills, Knowledge & Abilities · Understanding of chronic disease processes and patient self-management principles. · Familiarity with managed care workflows and interdisciplinary care coordination. · Proficiency in Microsoft Office (Word, Excel, Outlook, Access, PowerPoint). · Ability to type 40+ words per minute with accuracy. · Strong communication, organizational, and critical-thinking skills. · Ability to work collaboratively and escalate clinical concerns appropriately. PHYSICAL, MENTAL & ENVIRONMENTAL REQUIREMENTS The physical, mental, and environmental demands described here are representative of those required to successfully perform the essential functions of this job. This role requires extended periods of sitting, computer/phone use, and occasional walking, standing, and light lifting. Travel between sites may be required. Must be able to maintain focus in a fast-paced setting, adapt to changing priorities, and handle sensitive patient information with confidentiality and professionalism.
