JOB SUMMARY

The Care Coordinator will oversee day-to-day care coordination, manage client cases, ensure effective communication between caregivers and clients and provide exceptional customer service.

QUALIFICATIONS

• Experience in care coordination and case management
• Strong communication skills
• Familiar with home care procedure.
• Ability to work independently and collaboratively.
• A compassionate approach towards patient care with a commitment to improving health outcomes.
