JOB SUMMARY

The Care Coordinator plays a vital role in ensuring that clients receive the highest quality of care and support. Responsibilities
  • Coordinate and manage client care plans, ensuring all services are tailored to individual needs.
  • Facilitate communication between clients, families, and healthcare professionals to ensure seamless service delivery.
  • Maintain accurate records of client interactions and care plans within the office system.
  • Schedule appointments and follow-ups for clients with healthcare providers as necessary.
  • Monitor client progress and adjust care plans in collaboration with healthcare teams.
  • Provide support to clients and their families by answering questions and addressing concerns regarding care services.
  • Assist in the development of educational materials for clients regarding available services and resources.
  • Participate in team meetings to discuss client cases and improve service delivery.
