Job DetailsJob Location: Blakeford at Home - Nashville, TN 37215Position Type: PRNTravel Percentage: Road WarriorJob Shift: DayJob Category: TransportationJob Summary: Provide safe and timely transporation to an from scheduled appointments and accompanies clients during an appointment. Reports accurate findings from the doctor and changes in the plan of care to nursing staff and/or family members. Responsibilities: Serve as companion to clients. Communicate with clients throughout the visit. Encourage and assist clients in participating in appointments. Accompany clients to scheduled appointments as needed. Assist them in and out of the vehicle and with ambulation throughout appointment. Serve as driver to clients, safely transporting them to and from appointments throughout the community. Respect client privacy and confidentiality during transportation. Maintain accurate records of trips, mileage, fuel use, and any incidents or issues during travel. Report incidents, changes in client needs and any other observations to the supervisor. Represent the Company with professionalism and upstanding behavior in all interactions with clients. This role is PRN; Monday -Fridays. QualificationsEDUCATION & EXPERIENCE: High school diploma or GED; or one to three months related experience and/or training; or equivalent combination of education and experience. SKILLS & ABILITIES: Ability to drive in a variety of weather and traffic conditions Good driving record/history Reliable transportation Ability to assist clients in and out of car, set up walker, push a wheelchair at least a 100 ft. lift a minimum of 20 pounds.
