Responsibilities

ABOUT THE ROLE…

As a Recovery Coordinator you will be an integral member of the dynamic multi agency team developing and delivering comprehensive and tailored individual interventions that will include delivery of a full range of structured, psychosocial and clinical support. The post calls for close co-operation with other professionals such as GP’s, practice nurses and pharmacists and will involve delivering clinics from host environments/GP practices.

Key duties of the role will include:

  • Working autonomously to manage a caseload of service users, including delivering a range of psychosocial interventions.
  • Working as part of a multi-disciplinary team to undertake ongoing assessments, risk assessments, and appropriate referrals within the community.
  • Facilitating group sessions with a range of individuals of varying backgrounds.
  • Working with partnership agencies to ensure continuity of care and support.
  • Providing duty cover for individuals accessing our services.
