QUALIFICATIONS

  • High school diploma, GED, or an acceptable combination of experience and education.
  • Good verbal and written communication skills.
  • Valid Oregon driver’s license.
  • Excellent driving record.
  • At least 18 years of age.
  • Pass criminal history check & drug screen, including THC.
  • Must obtain CPR & First Aid Certificate.
  • Proven, stable employment history with good work ethic, attendance, and punctuality.

Responsibilities

RESPONSIBILITIES

  • Serve as a role model for appropriate work behaviors, social skills, respect for others, and cooperation.
  • Follow all trainee protocols, including training, employment, medical and behavioral supports.
  • Provide transportation to trainees as needed.
  • Complete and provide evidence of trainings to meet state requirements for Direct Support Professionals.
  • Keep relationships with trainees professional and limited to work settings and supports. Notify your supervisor of any conflicts of interest due to preexisting relationships with trainees.
  • Client supervision.
  • Driving throughout Douglas County.
  • Teaching appropriate behavior, dress, and speech.
  • Compile reports regarding aspects of the client’s day.
  • The position fosters positive relationships between the community, Sunrise, and our clients.

ESSENTIAL FUNCTIONS

  • Participate in transportation duties (transportation for participant activities and/or Meals on Wheels).
  • Assist individuals with disabilities to acquire and develop those skills, behaviors, attitudes and personal characteristics that will assist them to achieve self-fulfilling lives.
  • Provide peri-care for all individuals as needed.
  • Provide customer assistance to donors and retail department dropping off, delivering, or picking up thrift goods.
  • Assist supervisor and/or DSA Services Coordinator in providing case management of program participants as directed. This would include goal setting and training that would maximize their potential.
  • Provide support and assistance to supervisor, other DSPs, Services Coordinators, participants, volunteers, and all other staff in the DSA programs.
  • Comply with all applicable federal, state, and local rules and regulations.
  • Do not exceed responsibility/authority that has been delegated by supervisor.
  • Perform job and exhibit good judgment in situations not specifically covered by agency policy and/or procedures.
  • Complete training requirements as assigned by department.
  • Learn new skills/methods and perform duties as requested by the supervisor in appropriate time frame.
  • Report any incidents of neglect, abuse, emergencies or problems to the supervisor and/or appropriate person per policy.
  • Help develop, implement, and monitor Individual Support Plans (ISPs).
  • Help to maintain and monitor agency records and case files in a professional manner consistent with prescribed guidelines and expectations; e.g. organize, collect data, monitor documentation of participants’ time off requests, case notes, ISPs, time records, and additional information required.
  • Keep relationships with clients professional and limited to work settings and supports. Notify your supervisor of any conflicts of interest due to preexisting relationships with trainees.
  • Submit time sheets and other required documentation in a timely and accurate manner.
  • Attend and participate in required meetings, trainings, and committees.
  • Assure quality of services provided to participants.
  • Maintain a safe work environment.
  • Serve as a role model for participants and other staff.
  • Observes scheduled work hours and days.
  • Other related duties as assigned.
