As the Elder Care Case Manager, you will play a pivotal role in individuals’ lives, making a significant impact through:

  • Receive and respond to all calls for elder care services.
  • Conduct in‐home assessments using program‐specific tools to identify clients’ needs, including health, disability, wellness, financial, housing, family, legal, local resources, and advocacy.
  • Collaborate with clients to develop individualized service plans outlining clear goals and objectives to support independent living.
  • Assist clients directly or through referrals to access needed services, such as in‐home care, legal aid, or community resources.
  • Provide short‐term in‐home counseling to address life‐changing events and needs, offering education and advocacy to clients and families about services and systems.
  • Conduct regular in‐person or phone visits to ensure client safety and needs are met, acting as a liaison for families as needed
  • Document all client interactions, collateral contacts, and service plans in client records and program databases, maintaining strict adherence to HIPAA regulations and agency confidentiality standards

Elder Care Case Manager Minimum Salary: $ 22
Elder Care Case Manager Maximum Salary: $ 22
Elder Care Case Manager Schedule: Mon-Fri (25 Hours) Flexible within office hours

Qualifications:

  • Bachelor’s Degree in Social Work or related field
  • 3-5 years experience working with the senior population
