Description Holyoke Medical Center is currently recruiting for a Emergency Department Pharmacist. The successful candidate will provide safe and effective pharmaceutical care in an integrated model, which includes operational and direct patient-centered services in collaboration with physicians, nurses, and other hospital personnel. Activities performed in compliance with the hospitals' mission statement and the policies, procedures, and goals of the department, the hospital and regulatory agencies. This position reports directly to the Pharmacy Clinical Manager and practices under the general direction of the Director of Pharmacy. Evening shift hours M-F 3-11pm. Essential Job Functions Completes all required annual competencies. Attends departmental meetings and mandatory in-services/updates. Contributes to obtaining departmental and institutional goals, and participates in quality improvement initiatives. Enhance hospital pharmacy knowledge through a minimum of 15 hours of continuing education credit annually. Some of which may also meet annual elective competency requirements. Obtains and maintains BLS & ACLS certification. Medication Reconciliation. Responsible for supporting quality initiatives through data collection, management, analysis and participation in improvement initiatives. Participates in pharmacy operations and medication dispensing. Provides pharmaceutical services throughout the medical center. Participates in all aspects of the medication-use system while providing comprehensive and individualized pharmaceutical care to the patients in the emergency department. Utilize effective interpersonal communication skills to maintain high-quality inter- and intradepartmental working relationships. Keeps supervisor abreast of activities within area of responsibility and communicates needs on a timely and routine basis. Use time constructively and organize assigned work for optimal productivity. Perform other work-related duties as assigned. Current registration and unencumbered license as a pharmacist with the Massachusetts Board of Registration in Pharmacy – required. Board Certification through the Board of Pharmacy Specialties as approved by the Clinical Manager or Director of Pharmacy. ACLS Certification for Clinical Pharmacist - required. Hospital experience preferred. Demonstrated strong computer skills required.
