JOB SUMMARY Work readiness, assessment, evaluation, support, education, and task-oriented interventions aimed at skills enhancement and job attainment. RESPONSIBILITIES Reasonable accommodation may be made for individuals with disabilities to perform essential job functions. * Provide case consultation, crisis intervention, discharge placement, individual and group work, and assist clients in meeting basic needs and employment needs. * Complete accurate and timely documentation as contractually required and meet minimum requirements for direct billable client services. * Advocate for clients to ensure work success. Assist clients with linkages to income and benefits.   * Educate clients about competitive work opportunities, assessing interest/work needs as indicated. * Offer direct placement of clients in positions appropriate to skill level and personal needs. * Provide expertise to the team in areas related to work trends, employment models, potential employer profiles, and demographic data as needed. * Provide long-term job coaching for clients to assist in work stability and retention. * Ongoing client support, providing information and education to family and other supports. * Facilitate placement including pre-placement visits and transportation as needed.   * Shift coverage including part of the on-call team. * Provide field-based services as required by program and client need. * Maintain confidentiality of client’s care. * Participates in scheduled program meetings. * Other relevant duties as assigned. QUALIFICATIONS * Bachelor’s degree and 1 year experience or associate degree and 3 years' experience or 5 years' experience in related field required.  Need to verify this is required! * High School Diploma or GED required. * Knowledge of employment needs of individuals with behavioral health needs preferred. * Cultural competency understanding required. * Bilingual preferred. * Knowledge of Microsoft Office and EHR software preferred.
