Encompass Care Coordinator  * Salary Range: $24.04-$33.66 per hour, based on relevant years of experience * Location: Larimer county, Colorado – Orchards and Remote * Status: part-time, 20 hours per week * Schedule: Weekdays, some evenings required  The salary range for this position is SummitStone’s good faith belief and reasonable estimate of the range of possible compensation for this position at the time of the posting. The compensation offered to the successful applicant may vary based on factors including the applicant’s education, training, skills, experiences, and other job-related information and how relevant those are to SummitStone’s needs, aspirations, organizational priorities, and the evolving needs and delivery of services to the community that it serves.   Role Overview:   The First Step Care Coordinator works on a clinical team that provides assessment and brief clinical services to clients aged 12 years and older seeking mental health and substance use treatment. The Care Coordinator provides direct case management and outreach services for clients as needed, coordinates with clinical staff in completing referrals and connecting clients to services both internal and external to SummitStone, and supports program tracking and data collection efforts, including the completion of state reporting documentation. The ideal candidate for this position will be people-oriented, motivated and take initiative, have strong communication skills while interacting with clients, staff, and community partners, and be detail-oriented and willing to follow and develop essential workflows.   Essential Duties:  * Provide direct support to families in navigating their behavioral healthcare. * Coordinate and communicate as needed with other SummitStone staff and departments, including intake, operations, medical, and clinical teams. * Provide case management to families experiencing challenges in accessing community resources.   * Complete internal and external service referrals for clients, providing follow-up and support to clients in connecting with those referrals. * Maintain tracking spreadsheets and complete other data collection efforts to support program goals and development. * Work with clinical staff to complete state reporting documentation.  * Perform other duties as assigned. Required: * Bachelor’s degree in psychology, social work or related field from accredited College or University.   * 1-3 years minimum directly relevant experience in behavioral health field. * Experience in coordinating services with other agencies.    Licenses and Certifications: * Valid Colorado driver’s license or the ability to obtain one within 30 days of hire required; must maintain a clean driving record.
