ABOUT THE OPPORTUNITY

Are you a compassionate and experienced nurse with a passion for End of Life Care? We are seeking a dedicated End of Life Care Coordinator to join our multidisciplinary team. In this pivotal role, you will provide expert clinical leadership and coordination of care for patients approaching the end of life, ensuring dignity, comfort, and support for both patients and their families. You will work closely with medical, nursing, and allied health professionals to implement best practice models of care and facilitate education and support across the facility.
As a Clinical Nurse Specialist Grade 2, you will bring advanced clinical knowledge, strong communication skills, and a commitment to person-centred care. Your ability to lead quality improvement initiatives and mentor staff will be key to enhancing the delivery of end-of-life care across our health service. Responsibilities

The Clinical Nurse Specialist Grade 2 End of Life Care Coordinator (CNS2 EoLCC) will provide safe, compassionate, quality, person centered specialist care to support the delivery of End-of-Life Care (EoLC) across South Western Sydney Local Health District (SWSLHD). The position will collaborate with patients, families, carers, and a multidisciplinary team to achieve specific goals and outcomes. The role also includes providing telehealth consultations to SWSLHD sites that lack EoLC services, especially during weekends. The CNS2 EoLCC will review current practice, assist in introducing evidence-based models of care such as the last days of life toolkit, Advance Care Planning (ACP) and Voluntary Assisted Dying (VAD).
