You will provide a high standard of clinical assessment, treatment and secondary consultation to clients and staff of the St. Vincent’s integrated adult MHS, demonstrating excellence in discipline specific and recovery-focused skills. The adult mental health service components are: Footbridge Community Care Unit (CCU), a 44 bed Acute Inpatient Service (AIS) located on the St. Vincent’s Hospital site, the North Fitzroy PARC and two community mental health services (CMHS) at Hawthorn and East Melbourne (Clarendon CMHS). Community mental health services comprise the Community Acute Response Team (CART, Triage and Crisis assessment and Treatment Service (CATS), Extended Triage (ET), Emergency Department Mental Health (EDMH), PACER, Police support and HOPE (Suicide Prevention). Other services include Continuing Care (CCT), STAY Housing Support, Mobile Support and Treatment (MST) and Homeless Outreach (CHOPS) services. A Consultation & Liaison Service is provided to the health service. NEXUS Dual Diagnosis Service and the Body Image and Eating Disorders Assessment and Treatment Service (BETRS) are regional services. Statewide services are the Victorian Dual Disability Service and the Victorian Transcultural Mental Health. The Acute Inpatient Service includes 5 beds designated as the Koori State-wide Inpatient Service and linked to the Victorian Aboriginal Health Service. POSITION PURPOSE The Clarendon Continuing Care Team provides ongoing specialist treatment and support to meet the identified needs of clients who suffer with a serious mental illness and associated high level of disability in the City of Yarra. Intervention includes a range of clinical and therapeutic interventions using the ‘Strengths’ model of individual case management. Under the guidance of the Team Manager and Team Consultant Psychiatrist, and in conjunction with team members the Enhanced Care Clinician will provide and coordinate short to medium term additional specialist, targeted therapeutic interventions to consumers that are receiving case management services. Interventions provided could include, but are not limited to, tapered assertive engagement and support to boost consumer autonomy and accessibility, formulation-driven evidence-based psychotherapy (e.g., ACT, DBT, CBT), providing a comprehensive range of psychological assessments including diagnostic and personality assessments, family therapy, peer support, interventions focused on activity/vocation undertaking risk assessment and safety planning for high risk consumers (e.g. developing service-wide management plans to co-ordinate crisis responses), additional treatment interventions to help formulate and meet holistic physical health or dual diagnosis needs. POSITION DUTIES Work within the Strengths Model of recovery orientated practice. Build a therapeutic alliance with identified consumers and actively engage them in the planning, and delivery of their care. In collaboration with the treating team, undertake the assessment, coordination and provision of treatment and delivery of targeted evidence-based interventions to identified consumers with complex mental health related needs in line with SVMH policies and strategic direction. Utilise discipline-specific skills as appropriate and as required by consumers, their family/carers and the Service. Assist in the provision of primary, secondary and tertiary consultation services. Display active participation in professional development and ongoing learning opportunities. Attend and participate in discipline specific meetings and display a commitment to attending discipline specific training. Work collaboratively within the multi-disciplinary team. Ensure all reporting information is coordinated and completed as required. Maintain all documentation in line with SVMH policies, including clinical documentation, outcome measures and CMI reporting SELECTION CRITERIA ESSENTIAL REGISTRATION, LICENSE OR QUALIFICATION REQUIREMENTS Registration as a General Psychologist with AHPRA.
