Job Title: Enrolled Nurse – Care Coordinator (Full-Time)
Location: Bundall, Gold Coast QLD
Work Type: Full-Time | Monday to Friday, 8:30am – 4:30pm
Remuneration: Competitive base salary + superannuation
Travel Region: Gold Coast South to Northern Rivers

Right at Home Gold Coast is seeking a dedicated and experienced Enrolled Nurse – Care Coordinator for a full-time, onsite role based in our Bundall office. ABOUT THE ROLE:

In this key position, you will coordinate and oversee the delivery of safe, high-quality, and person-centred care for our aged care and disability clients. You will be responsible for maintaining a caseload, performing clinical assessments, and ensuring care aligns with client needs, care plans, and available funding.

KEY RESPONSIBILITIES:

• Conduct comprehensive client assessments and develop care plans
• Maintain compliance and manage a caseload of clients
• Coordinate care and services within budget and funding allocations
• Perform administrative duties related to client care coordination
• Liaise with clients, families, and internal/external stakeholders
