POSITION OVERVIEW:

  • Type: Permanent, Part-Time - 32 hrs./FN
  • Hours: Mon & Friday 0700- 1430 (7 hrs.) and Alt Sat 0700-1100 (4 hrs.)
  • Location: Serving Ballarat and surrounding areas
  • Travel: Must be willing to travel for client care

Responsibilities

ROLE RESPONSIBILITIES & QUALIFICATIONS

  • Provide health promotion and education to clients and their carers
  • Promote and deliver our award-winning Wellness for Independence® programs, both in-person and virtually
  • Monitor and improve health outcomes through Telehealth.
  • Direct Care Services: Provide care across various funding programs, including wound care, diabetes management, medication management, stoma care, and catheter care.
  • Centered Care: Assess, develop, and evaluate nursing care plans collaboratively, focusing on client goals and independence with support from Allied Health clinicians.

WHAT YOU NEED TO BE CONSIDERED FOR THE ROLE?

  • Understanding of community nursing and primary health care principles, including working within a consumer-directed care framework
  • At least 1 year of experience in community, acute, or aged care
  • Experience working independently or as part of a team
  • Strong organizational skills and attention to detail
  • Proficiency with electronic devices and client management systems (smartphones, tablets, laptops, email, etc.)
  • Current Registration with the Australian Health Practitioner Regulation Agency (AHPRA)
  • Experience providing advanced clinical care, including chronic disease, medication, palliative care, catheter management, stoma care, wound care, and diabetes management
