Responsibilities

ABOUT THE ROLE

This Enrolled Nurse role will be working within a multidisciplinary Transitional Aged Care Program (TACP) team, providing case management under the supervision of Registered Nurse.
The Aged Care Division delivers a range of clinical and non-clinical services across the Western NSW Local Health District to support older people in their families to enable them remain supported in their home.
The Aged Care teams and programs have a strong commitment to advocacy, restorative care and reablement back to wellness and independence. The position will enjoy cooperation and support within a strong leadership team and can expect significant challenges in a changing Aged Care environment which needs to respond to a growing patient population and Commonwealth and State Health reforms impacting on operations.
