JOB DESCRIPTION

Registered Nurse (RN) in Home Aged Care Position Description: The Registered Nurse (RN) provides high-quality nursing care to elderly clients in their homes, working collaboratively with healthcare professionals and the team to support the health, dignity, and well-being of clients in accordance with Australian aged care standards.

*QUALIFICATIONS: *

  1. Current EN/RN licensure with the AHPRA.
  2. Previous nursing experience, preferably in aged care or community health settings.
  3. Strong clinical assessment skills and knowledge of nursing practices related to elderly care.
  4. Excellent communication and interpersonal skills, with a focus on building rapport with clients and families.
  5. Ability to work independently and as part of a multidisciplinary team.
  6. Compassionate, patient-centred approach to care, with a commitment to promoting the dignity and quality of life for clients.
  7. Drivers license and vehicle with relevant registration and insurances.

Responsibilities
  1. Conduct comprehensive health assessments of elderly clients to evaluate their physical, emotional, and psychosocial needs.
  2. Monitor vital signs and assess clients’ health status, identifying any changes or potential complications in accordance with aged care best practices.
  3. Develop individualised care plans based on assessment findings, client needs, and goals, ensuring alignment with the Aged Care Quality Standards.
  4. Collaborate with clients, families, and interdisciplinary teams to ensure care plans are understood and followed, promoting client involvement in their own care.
  5. Administer medications and treatments as prescribed by healthcare providers, ensuring compliance with medication management protocols.
  6. Perform medical procedures such as wound care, catheter insertion, and other nursing interventions as needed, following infection control guidelines.
  7. Educate clients and their families about medical conditions, treatment plans, and self-care techniques to promote understanding and compliance. Educate and train support workers on client’s safety and wellness.
  8. Provide guidance on medication management, nutrition, and lifestyle modifications to support overall health and well-being.
  9. Collaborate with other healthcare professionals, including general practitioners, allied health providers, and social workers, to ensure comprehensive and coordinated care.
  10. Facilitate communication between clients, families, and the healthcare team regarding any changes in health status or care needs.
  11. Maintain accurate and up-to-date documentation of client assessments, care plans, interventions, and outcomes in compliance with Aged care Safety and Quality Commission regulatory standards.
  12. Record changes in client conditions and report significant findings to appropriate healthcare providers in a timely manner.
  13. Advocate for clients’ rights and preferences, ensuring that they receive the care and support they need to make informed decisions about their health and well being.
  14. Assist clients in navigating the aged care system and accessing community resources and support services.
  15. Respond to medical emergencies in the home, providing appropriate care and intervention as necessary while adhering to emergency protocols.
  16. Develop and implement emergency preparedness plans tailored to each client’s specific needs and circumstances.
  17. Participate in quality improvement initiatives to enhance the quality of care provided to clients in home aged care settings.
  18. Stay informed about best practices, evidence-based guidelines, and regulatory changes in aged care in Australia.
  19. Engage in ongoing education and training to maintain licensure and enhance nursing skills relevant to home aged care.
  20. Stay current with advancements in medical technology, treatments, and care practices to improve service delivery.
