Description

Develop quarterly health topics to address on LHL Program bulletin boards. Create strategy around important healthcare Awareness Days related to LHL. Create educational handouts for LHL programs – for staff use and patient use. Collaborate with other members of the Lending Hands for Life team to coordinate and facilitate LHL Consumer Advisory Council (CAC). Collaborate with other members of the Lending Hands for Life team to Coordinate and facilitate LHL’s three monthly Patient Support Group Meetings for HIV+ patients. Collaborate with other members of the Lending Hands for Life team to coordinate and facilitate LHL’s monthly Patient Support Group Meetings for PrEP patients. Plan and execute agency-wide World AIDS Day Programming. Assist with AIDS Run/Walk. Conduct quality improvement and evaluation measures on patient health outcomes related to screening completion rates, access to care and health outcomes for HIV, PrEP and Hep C programs. Supports with registry of Erie’s patients with Hepatitis C. Work with the LHL Coordinator to assist with patient PrEP care, including intakes, appointment follow-ups, reminder calls, and no-show follow-up.Create PrEP program materials for use by providers, Patient Navigators, patients, etc. Provide support with HIV, PrEP, and HCV intakes. Enroll patients in ADAP and pharmaceutical manufacturer assistance programs for HIV+, HCV+, and high-risk HIV- patients. Administer Rapid HIV tests - simple finger prick method and conduct pre and post-test counseling with clinic walk-in patients and community members. This position is located at our Humboldt Park location, 2750 W North Ave, Chicago, IL 60647.
Member Duties : The member will serve closely with LHL, serving HIV-positive patients, patients with Hepatitis C, and high-risk HIV- patients, and implementing patient education and program improvement initiative
