We have an exciting opportunity to join the transitions team as an experienced social worker. In this role, you will be responsible for managing a caseload, supervising assistant care managers and link workers as well as sharing the transitions duty manager role.
Within the Transitions team, we work closely with young people between the ages of 14 years and 25 years who may have a learning disability, neuro diverse diagnosis, physical disability or mental health needs and support them to prepare to enter in to adulthood and develop their independence as much as possible. This enables you, as a professional to get to know a young person and their family over a longer period of time. You will be responsible for completing the Care Act Assessment in preparation for the young person turning 18 years old.
During this time, you will be working in a multi-disciplinary framework with education, health, children services and supported employment services amongst others before the young person transitions to Adult Social Care. Following this, you will follow the care management process to review the support plan annually as well as continual planning for the future with the young person and their family.
You will be responsible for managing a caseload that may include capacity assessments, completing DOLS applications and attending MDT meetings. You will also work with safeguarding alerts, acting as enquiry manager for those within the team.
You must be fluent in the English language (as a requirement of Part 7 of the Immigration Act “for the effective performance of a customer-facing role”).
You must hold a formal social work degree and be Social Work England registered to apply for this post.
