We’re seeking an experienced Support Coordinator to join our close-knit team in Parramatta. This role is more than coordination — it’s about walking alongside people with disability, amplifying their voice, and helping them navigate the NDIS with clarity, confidence, and care.

Responsibilities
  • Build genuine, trusting relationships with participants, families, and support networks
  • Help participants understand their NDIS plan, funding, and available supports
  • Connect clients with appropriate services, allied health professionals, and community supports
  • Support psychosocial recovery and goal achievement through tailored care planning
  • Facilitate assessments to determine funding needs (e.g. complex home modifications)
  • Coach and empower clients to build confidence and make the most of their plan
  • Maintain regular communication and respond to feedback with a commitment to improvement
  • Prepare and attend plan review meetings, including reports and goal setting
  • Uphold confidentiality and adhere to all organisational policies and WHS standards
