Join Benchmark as an Extended Services Case Coordinator to provide support to individuals in sustaining long-term employment following the closure of their vocational rehabilitation case. Must be willing to travel the greater Fort Wayne area as well as Marion. Conduct regular meetings with individuals and employers on-site. Offer support in navigating workplace dynamics and fostering relationships. Facilitate re-referrals to vocational as necessary. Provide outcome data to designated personnel. Monitor and summarize monthly billing activities. Perform periodic file reviews for accuracy and timeliness. Coordinate all business between department and state funding bodies. Oversee client programming and services. Adhere to attendance requirements set by the program, Benchmark, and individual clients. QUALIFICATIONS: Bachelor's degree in a related field and/or one (1) year experience preferred with people with disabilities Three (3) years experience of continued and verifiable employment within two years of submitting an application Valid Indiana Driver’s license and auto insurance Reliable transportation Ready to Join Us?
