We are seeking a compassionate and organized Extended Services Case Coordinator who will also provide direct job coaching support. This role supports individuals with disabilities in maintaining successful employment by providing ongoing case management, employer coordination, and on the job coaching. Meet with EXT/SEFA individuals and employers at the job sites a minimum of two times each month. Provide hands-on job coaching to support individuals in developing workplace skills, overcoming job-related challenges, and maintaining successful employment. Make re-referrals to Vocational Rehab as needed. Monitor and summarize monthly billing. Conduct periodic file reviews for timeliness and accuracy. Coordinate all business between the department and state funding agencies. Coverage counties: St. Joseph and Elkhart Other duties as assigned. Requirements: Valid driver’s license, auto insurance, and reliable transportation required. Bachelor’s degree in a related field preferred and/or at least 1year of experience working with individuals with developmental disabilities 3 years of continuous, verifiable employment within the past 2 years of application submission.
