Pride in North Carolina, LLC is seeking a Facilitator for our High Fidelity Wraparound program to engage high-need youth and families in developing and monitoring individualized care plans. Position Summary: The Facilitator ensures the principles and phases of the High-Fidelity Wraparound process are delivered with fidelity to the model, supporting families in achieving positive outcomes. Key Responsibilities: Engage youth and families in care planning and monitoring. Develop integrated plans and natural support systems. Maintain strong relationships with families and community partners. Ensure fidelity to High-Fidelity Wraparound principles. Document all activities accurately and timely. Education & Experience: • Required: Bachelor’s degree in human services or related field (experience may substitute). • Two years of experience providing services for children, youth, and families.
