Description

Facilitators work with people with disabilities and their families to create strong and engaged support networks of caring relationships. Facilitator explores with the individual their gifts, interests, and current routines to help create, build, and give direction to the network. The Facilitator’s role is to create opportunities, nurture relationships, and explore the community with the individual receiving the support.
Individual must be highly organized and able to remember information, create schedule, keep detailed records such as timecard and communication notes, and be able to write narratives, goals, and plans demonstrating basic understanding of grammar and sentence structure.

QUALIFICATIONS:

· Must be at least 18 years of age
· AA degree in field directly related to developmental disabilities or teaching and/or
· A minimum of 12 months of experience as an Instructor, Support Staff, or experience in a like position working with disabled adults
· Must have a valid California Driver’s License and proof of auto insurance
· Must provide a clean DMV report
· Must meet all pre-employment requirements prior to becoming an employee

Responsibilities

· Treat all clients with respect and dignity
· Use the ‘Building a Network Strategy’ as a guide to create a written network development strategy
· Collaborate with the person and their networks to identify priorities and design workplans
· Spend up to 10 hours per month with each person and their network implementing the workplan
· Demonstrate effective strategies for helping people to build relationships, make plans for their life and find meaningful ways of being involved in their communities
· Respond to feedback and concerns shared by the Person at the Center of the network, their family and network members
· Oversee communication within the network by responding to all phone calls and emails in a timely manner as well as providing monthly updates in a format the network decides, i.e. newsletter, social media, group email, etc.
· Documentation of progress/needs/barriers through communication logs, summary reports, incident reports and monthly updates.
· Interact professionally with relatives, significant others and agency staff in clients lives, including network members, services coordinators, conservators, payees, landlords
· Plan meetings and gatherings with the network members with at least one formal meeting and one informal gathering every 6 months, based network preference
· Complete documentation in a timely manner
· Participate in staff meetings, in-services/continuing education requirements, team meetings, etc.
· Daily notification to office personnel of work schedule
· Demonstrate good reading, writing and communication skills.
· Adhere to all due dates for paperwork, trainings, auto renewals, etc.
· Other duties as assigned
Job Type: Part-time
Pay: $20.00 - $22.00 per hour
Expected hours: 8 – 24 per week
