Description The Facility Based Outing Instructor provides assistance to program members to achieve outcomes and action steps from their individualized service plan in the facility or community settings. Additionally, the Facility Based Outing Instructor provides assistance to individuals who need help with daily living skills, including toileting, eating, transferring, meal preparation, bathing and walking. The Facility Based Outing Instructor is also responsible for providing a safe environment. Pay is $14.00 Per Hour Duties and Responsibilities Essential Duties and Responsibilities. The following list is not meant to be all-encompassing. There are many individual responsibilities and tasks that this position requires. Additional duties may be required, depending on the needs. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Use person-centered practices in daily tasks. Implement daily schedule. Assist program members with completing outcomes and action steps in service plan through various community and facility activities. Transport program members and follow Easter Seals’s transportation policies and procedures and vehicle operating procedures. Maintain appropriate documentation and required program data (daily notes and implementation strategies), and turn in on time. Involve program members and provide supervision in community activities and/or facility based activities. Teach and record goals and outcomes that are to be implemented in the facility or community. Assist program members with eating, toileting, dressing, or other self-care needs as appropriate or instructed. Maintain a safe and positive work environment. Carry out therapeutic goals as prescribed by licensed therapist. Maintain a safe and positive work environment. Ensure program members are not left unsupervised in cars, vans, home or community based settings unless it is specified in their service plan. Adhere to all safety practices, policies and procedures. Administer medications to program members in accordance with Easter Seals medication administration policy, as assigned. Other Duties and Responsibilities Inform the Day Services Manager as individual program needs change. File forms and records as required. Perform other related tasks as required. Maintain the upkeep of the vehicle and report safety issues to the responsible person. Perform other related tasks as required. Requirements Position Requirements Position Minimum Requirements At least 21 years of age Current, valid Tennessee driver’s license and an acceptable driving record. Education and Training High school diploma or equivalent math and reading comprehension skills. Ability to complete all required training. Job Experience Stable employment history. Skills and Competencies Basic computer skills. Ability to communicate clearly and effectively with program members and staff both verbally and written. Ability and desire to work with individuals with disabilities. Ability to use good judgment. Ability to follow specific written and verbal instructions
