Description

The Newport Community Resource Team have an exciting opportunity for a motivated Falls Practitioner working within their Specialist Falls service. You will have a Health, Social Care or Paramedic professional qualification.
The position requires excellent assessment skills together with a history of effective multi-disciplinary team working. The successful candidate will undertake falls assessments and programmes with individual’s in their own homes whilst also being part of an established and successful Consultant led Falls Clinic team.
The Newport Falls Service is integrated with the wider Community Resource Team and partners. We work closley as a multi-disciplinary service to ensure a full hostilic assessment is completed for the individual to help minimise the risk of falls and increase independence. The service also offers the successful candidate excellent learning and development opportunities.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
