Description

Due to retirement an exciting opportunity has arisen for a motivated, adaptable health care assistant/therapy assistant to join the friendly Falls Prevention Team, who work agile across Bradford and Airedale.
We are looking for someone who is enthusiastic, organised and flexible with excellent communication skills to join our proactive team. You will be required to work independently under the supervision of the Band 5 and Band 6 leads, demonstrating excellent interpersonal skills and a positive attitude to anticipatory care and falls prevention.
The Falls Prevention team deliver a community based integrated model of healthcare for individuals who are identified at risk of falls. Team members deliver a blend of preventative and recovery focused interventions to support frail older adults in their own home.
This is an exciting opportunity as the team are currently introducing new ways of working to enhance the programme of care.

  • To work as part of the Falls Prevention Team across the geographical area of the organisation
  • To work as an effective team player contributing to the development of the service in collaboration with colleagues and senior team members
  • To provide a high standard of care that is research/evidence based and clinically effective at all times
  • To provide support, advice and education on falls prevention matters to colleagues, patients and carers
  • Provide knowledge and demonstrate a holistic approach to patient management across work areas, underpinned by falls assessment processes for patients who have fallen or who are at risk of falling.
  • To act as a positive role model at all times.
  • Develop one’s own knowledge and practice to meet objectives/changes in service.
  • In collaboration with the multidisciplinary team support staff to plan, implement, monitor and review patient care programmes in relation to falls prevention and bone health and health interventions for individuals.
