JOB SUMMARY

The Support Worker is responsible for gathering information, assessing family functioning issues, and developing intervention plans to address client needs. This role provides support, guidance, and problem-solving to family and youth, helping them with issues related to family dynamics, parenting skills, and emotional well-being. The position requires working closely with an integrated case management team and connecting clients to relevant community resources.

QUALIFICATIONS

  • Education: Bachelor’s degree in a related human or social services field.
  • Experience: At least one (1) year of recent related experience working with families and providing support services, or an equivalent combination of education, training, and experience.
Responsibilities
  • Client Assessment and Intervention Planning:
  • Gather relevant information regarding client problems, needs, and risks through interviews, observations, and meetings with caregivers and service providers.
  • Use inventories, checklists, and questionnaires to assess family functioning.
  • Develop and implement intervention plans in line with program guidelines to address identified issues.
  • Client Support and Problem-Solving:
  • Provide support, guidance, and problem-solving to clients in individual or group settings using active listening, conflict resolution, and basic group counselling techniques.
  • Facilitate psycho-educational group methods to resolve client issues related to family functioning.
  • Service Plan Development:
  • Participate in the development, modification, and evaluation of client service plans alongside the integrated case management team.
  • Attend regular case management meetings to report on clients’ activities, progress, and challenges.
  • Referral and Resource Guidance:
  • Provide clients with information on program services and refer them to relevant community service providers, resources, and professionals as necessary.
  • Parenting Skill Building:
  • Educate and support clients on parenting skills, including parent-child interaction, child development, discipline, guidance, and behaviour management.
  • Emotional Support and Feedback:
  • Offer emotional support and constructive feedback to clients, helping them navigate difficult family dynamics.
  • Educational Sessions:
  • Plan and conduct group educational sessions focused on topics related to family functioning, such as communication, parenting strategies, and conflict resolution.
  • Client Advocacy and Liaison:
  • Liaise with other community service providers, professionals, and school personnel to promote client interests and ensure coordinated service delivery.
  • Accompany clients to important meetings and appointments, advocating on their behalf when necessary.
  • Record Keeping and Reporting:
  • Maintain detailed records, statistics, and reports on client interactions and progress, and submit them to the supervisor/social worker as required.
