THE OPPORTUNITY:

We are looking for a passionate and skilled Case Manager to join our Family Based Care team, reporting to the Family Based Care Operations Manager. You will work with foster carers to provide trauma-informed, therapeutic case management, ensuring each child or young person is well supported and set up for success.
You will provide practical support and coaching to foster carers, help manage and stabilise placements, and advocate for the best outcomes for children and families. This role is ideal for someone with strong communication skills, an understanding of complex trauma, and a passion for making a lasting impact.

Some of the key accountabilities are:

  • Deliver high-quality case management, including regular home visits, needs assessments, and crisis support.
  • Build strong relationships with foster carers and provide therapeutic guidance, training, and advocacy.
  • Collaborate closely with the Department for Child Protection, service providers, and carers to ensure positive outcomes for young people.
  • Develop and monitor individual case plans, manage risks, and respond to incidents with creativity and care.
  • Support recruitment and onboarding of new carers and help strengthen community and interagency connections.
  • Participate in reflective practice, team development, and program improvement activities.

ABOUT YOU:

We are seeking a highly driven person who:

  • Possesses knowledge and experience in carer assessment, carer support and mandated training packages
  • Has a demonstrated understanding of appropriate psychoeducation for carers to assist them to respond to complex behaviours
  • Can demonstrate a clear understanding of supporting carers to understand child development
  • Has demonstrated experience and/or understanding of managing a caseload of clients
  • Possesses sound communication and interpersonal skills with an ability to prioritise your own workload
  • Holds knowledge and/or has experience working with young people under the Guardianship of the Chief Executive
  • Has an understanding of social justice principles – including equity of access, discrimination and equal opportunity, individuals’ rights and privacy
  • Holds a degree or tertiary qualifications in Social Work, Human Services
