Responsibilities

A Family Coach plays a key role in supporting persons with intellectual disabilities and their families through important life transitions, such as moving from school to community or between residential and community care. Based in MINDS Regional or Community Hubs, you will work closely with a multi-disciplinary team to develop and review Family Support Plans, while engaging caregivers to set goals, provide resources, and monitor progress. Using a coaching and partnership approach, you will help families build resilience and ensure that every transition is not only smooth but also sustainable.
