JusticeWorks YouthCare is seeking a Family Outreach Specialist / Family Advocacy Case Manager / Family Decision Support Coordinator! The Family Outreach Specialist / Family Advocacy Case Manager / Family Decision Support Coordinator plays a key role in locating, engaging, and building connections between children in out-of-home care and their relatives or other supportive adults. Through intensive outreach, relational mapping, and collaborative engagement strategies, this role supports the goal of permanency and emotional connection for every child.
As a Family Outreach Specialist / Family Advocacy Case Manager / Family Decision Support Coordinator you will be responsible for working with families to ensure they have access to the resources they need to support their children. You will be the point of contact for families, offering support and assistance with locating community resources. You will be responsible for assessing the family’s needs and providing resources and services to help families achieve stability.
You will be responsible for facilitating family-centered meetings and processes—such as Family Group Decision Making (FGDM), Team Decision-Making, and Permanency Planning Meetings—to bring together family members, caregivers, and professionals. The goal is to help families develop realistic, culturally responsive plans that address concerns and build on existing strengths. THE QUALIFIED FAMILY OUTREACH SPECIALIST / FAMILY ADVOCACY CASE MANAGER / FAMILY DECISION SUPPORT COORDINATOR WILL:

  • Engage the identified relatives, kin or significant individuals using a strength-based approach in order to develop a supportive network of connections for the youth or family.
  • Conduct outreach to engage families in services and programs.
  • Assess family needs through interviews and home visits.
  • Connect families to community resources (e.g., housing, healthcare, education).
  • Coordinate transportation, childcare, or other support services as needed.
  • Maintain case documentation and data for reporting and compliance.
  • Transport clients utilizing personal vehicle, as necessary.

THE QUALIFIED FAMILY FINDING COORDINATOR / FAMILY ADVOCACY CASE MANAGER / FAMILY DECISION SUPPORT COORDINATOR WILL HAVE:

  • Associate’s Degree required.
  • Possess computer literacy and knowledge of internet-based search tools.
  • Valid driver’s license, auto insurance and access to a vehicle.
  • Pass current, applicable clearances.
  • Pass pre-employment, post-offer drug screening.
  • Ability to work nights and weekends.
