About the role The purpose of this position is to lead a team of Family Partnership Workers providing a comprehensive home visiting service in partnership with the Nurse Home Visitors to women pregnant with an Aboriginal and/or Torres Strait Islander baby and their families. The position is integral to the successful implementation of the ANFPP. The Family Partnership worker contributes to the cultural acceptance of the program within the community and the maintenance of culturally safe visits to participating women and their families. This position supports the ANFPP team on a broad range of cultural issues and liaises with clients, family and community members. The Family Partnership Worker is responsible for maintaining high level standards of community practice, foster acceptance of the ANFPP model in the community and IUIH policies and guidelines. Your day to day duties may include: Work alongside ANFPP Nurse Supervisor assisting with day to day activities, including logistics, performance reviews and management. Provide support with reflective activities and Reflective Supervision for the Family Partnership Workers Builds and maintains relationships based on trust, support and growth with eligible women, their families, team colleagues and community members. Provides culturally safe home visits and support to women who are eligible for the ANFPP and their families. Plans and performs home visits in accordance with the clients stated goals, ANFPP model elements and ANFPP Home Visit guidelines. Ensure accuracy, maintenance and confidentiality of patient health records. Supports policies, procedures, guidelines and standards of ANFPP and IUIH. About You?
