POSITION SUMMARY The role of the Sherwood Park – Strathcona County PCN Family Practice Nurse is to provide comprehensive nursing services to the patient population of a family practice clinic within the Network. The nurse will work collaboratively with patients, family physicians and other team members to develop and implement care plans and provide follow-up with patients. The nurse will assess, manage and support patients to provide comprehensive care, education and guidance to patients with a variety of clinical conditions. Duties will vary depending on the clinic and patient population. 1.0 FTE Permanent Eligible for Benefits 9% RSP matching program Paid vacation and sick time Health & Wellness Spending Account Pay range, based on experience $28.66 - $37.76 Education support, including funding and paid time for professional development Schedule Requirements - this position requires flexibility to work scheduled shifts between Monday and Saturday*. Available shift options may include: - 9:00 AM - 5:15 PM - 10:45 AM - 7:00 PM - 11:45 AM - 8:00 PM *The successful candidate's final schedule will be determined based on operational requirements and their availability. KEY RESPONSIBILITIES Include, but are not limited to the following: Nursing skills/tasks will include injections, dressing changes, ear flushes, Pap tests, diabetes and prenatal education Work collaboratively within a family practice environment to enhance the delivery of primary care services Identify needs and set priorities with the patient, physician and other team members Provide assistance to physicians as required and appropriate to the position Assist in developing and implementing patient self-management programs Conduct assessments, initiate and implement management strategies and provide appropriate follow-up Provide relevant patient education to prevent and manage clinical conditions Assist patients to navigate through the health system Communicate with patients to determine the next appropriate health services required, based on presenting concerns and test results Provide direction in accessing appropriate local and regional health programs Assist in the development of health screening programs Implement strategies for improving the quality and provision of care for the practice population Initiate appropriate investigations Ensure the maintenance of accurate records in line with practice policy Participate in evaluation of the clinic and its services Provide training and technical support to new hires to accelerate their proficiency Other duties as assigned to maintain seamless patient flow throughout office visits EDUCATION & EXPERIENCE LPN designation with current registration with CLHA (previously CLPNA) in Alberta Current standard BLS Provider certificate Three years related nursing experience, primary care environment preferred Experience working in a team setting with other health professionals SKILLS & ABILITIES Strong knowledge of nursing processes and practices, including intervention strategies and therapies Strong, effective written and oral communication skills, high level of perceptiveness Ability to prioritize, manage time effectively and be flexible in a very active work environment Respect and understanding of confidentiality Customer service orientation Ability to manage a complex changing environment High levels of integrity and professional behavior OTHER Clinical experience in primary care nursing, community, gerontology and/or chronic disease management Knowledge in health promotion, health screening, assessment and education Comprehensive knowledge of normal laboratory values and diagnostic imaging Initiative, accountability, sound decision-making and communication skills Demonstrated ability and willingness to work in a dynamic environment that requires flexibility and balancing of competing priorities Experience in caring for patients with chronic, complex, and/or specialized needs Experience with Electronic Medical Records systems and good computer skills PERSONAL ATTRIBUTES AND COMPETENCIES Enthusiastic and positive can-do attitude Able and willing, in an unstructured environment, to be flexible, creative and juggle numerous competing priorities Ethical, responsible and accountable Respect other views/opinions Ability to have difficult conversations and be comfortable asking questions Exceptional understanding of and appreciation for confidentiality and privacy Ability to identify sensitive information OTHER OCCUPATIONAL REQUIREMENTS A current and clear Criminal Record Check, including a Vulnerable Sector Check, is required prior to start date A valid driver’s license and access to a vehicle for travel is required DISCLAIMER This job description indicates representative responsibilities and specifications of the position only and should not be considered to necessarily represent all responsibilities and specifications of the position.
