Summary An Advanced Practice Provider Resident (Nurse Practitioner or Physician Assistant) will commit to a training program based at STRIDE FQHC for the length of the contract. The structure of the program will include didactics, mentorship, graduated primary care patient load and panel, and primary care experiences. Upon completion of the program, the fellow will be a primary care provider well prepared for work in any urban FQHC or other primary care setting. Essential Duties and Responsibilities Practice family medicine at a clinic, serving all ages of community at large in primary care. Works under the advisement of the program and preceptors, who can give advice regarding care and career formation Attends didactics and completes assignments Participates in the program, evaluations, quality improvement projects, etc. And the following standard essential duties and responsibilities for every APP at STRIDE: Follows practice guidelines and standards of care. Provides direct patient care to patients of all ages- including adolescent health care. Evaluates and treats patients for acute and chronic concerns. Arranges referrals and consultation for patients with healthcare needs beyond the scope of practice. Communicates and coordinates with hospitals for transfer of care. Communicates and coordinates care with school-based health staff as appropriate. Offers clinical consultation and arranges referrals to other providers and clinical staff. Reviews labs, x-rays, and ultrasound reports and develops a plan of care when abnormal. Prescribes medications to patients, including counseling patients regarding the proper use and contraindications of medications as well as detect and diagnose adverse drug reactions. Evaluates medical charts to determine appropriate patient care follow-up and education regarding acute and chronic illnesses. Accurately and completely charts information regarding patient interactions into the Electronic Health Record (EHR) in a timely fashion. Works within and supports a team based and integrated care model, which includes medical assistants, other medical specialties, dental, behavioral health, patient navigation, and care coordination. Attends clinic meetings, specialty meetings, provider meetings, all staff meetings, and all other meetings as assigned. Helps determine topics of interest for specialty meetings to evaluate the quality and efficiency of patient care needs. Other duties as assigned. Compensation - $80,000, annually Residents are STRIDE employees with a salary of $80,000/year. Medical, Dental & Vision insurance coverage PTO and 7 paid holidays No nights, weekends or call Malpractice Coverage through FTCA Requirements Education and Experience Masters in nursing from an accredited nursing program/ Physician Assistant licensure and certification. Certificates, Licenses, and Registration Board certification. Licensed or eligible for licensure in Colorado. Active BLS/CPR certification. Knowledge, Skills and Abilities Demonstrated knowledge in primary care specialty. Ability to interact positively and build rapport with patients, coworkers and/or external contacts. Ability to demonstrate the capability to effectively and sensitively respond to the needs and concerns of the full range of STRIDE’s patient population. Ability to work independently, accept feedback and teaching, and organize work in a manner that ensures accuracy and efficiency. Ability to collaborate with other providers and clinical staff to develop continual quality improvement procedures. Ability to handle sensitive information ethically and responsibly. Ability to protect the confidentiality of patient, employee, and business information. Skill in using a variety of computer software including but not limited to the internet and MS Office Products. Knowledge of and ability to work with electronic health records (EHR) system.
