In this role, you will provide counseling, advocacy, and case management as part of an interdisciplinary hospice team, ensuring comfort, dignity, and holistic support throughout the end-of-life journey. Requirements Master’s degree in Social Work (MSW) from an accredited program (LCSW preferred) Minimum of 1 year of healthcare experience (hospice experience strongly preferred) Strong understanding of hospice philosophy and end-of-life care Excellent communication and interpersonal skills Current CPR certification Ability to meet annual influenza vaccination requirements Valid Colorado driver’s license, auto insurance, and reliable transportation Must meet company driving standards
