Job Summary: Work directly with individuals who are experiencing homelessness. Provide support and referrals to service providers to expedite clients’ opportunities to obtain employment and obtain and retain stable housing so they may become self-sufficient and end their homelessness.

Responsibilities

ESSENTIAL FUNCTIONS: IDENTIFY VULNERABLE INDIVIDUALS AND BUILD AN EFFECTIVE CASE PLAN FOR THEM CUSTOMIZED TO THEIR INDIVIDUAL NEEDS:

  • Conduct in-depth intake interview;
  • Review HMIS database for background information on returning clients;
  • Assess client to develop effective case plan;
  • Review case plan with client; provide resource information, referrals, and direction.
  • Perform other duties as required.

OTHER DUTIES:

  • Assist in training newly hired staff;
  • Build and sustain relationships with campus partners;
  • Perform other duties as required.
    Work Environment: This job primarily operates in a professional office environment however, occasional off-site venues and outside event areas in various weather conditions are expected. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
    Physical Demands: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk or hear. The employee is frequently required to stand; walk; use hands to finger, handle or feel; and reach with hands and arms. The employee must frequently lift and/or move objects up to 10 pounds and occasionally lift and/or move objects up to 25 pounds unassisted. The employee must be able to self-transfer and move throughout the facilities, between buildings, and around campus at multiple sites. Close visual acuity to prepare and analyze reports and data. Visual acuity and physical ability to operate a motor vehicle. The individual will need to sit at a desk for extended periods of time to accomplish tasks and must have the manual dexterity in order to use keyboard and mouse.
    Position Type and Expected Hours of Work: This is a full-time position. Days and hours of work are primarily Wednesday through Saturday with time allotted for breaks/lunch. Scheduled hours may vary depending on agency needs. Evening and weekend work may be required as job duties demand. Travel is primarily local during the business day.
