JOB QUALIFICATIONS:

  • Bachelor’s degree in Social Work, Psychology, Counseling or Human Services field.
  • Two years of experience working with children, youth, and families.

Responsibilities
  • Ensure the safety of all children served. Ensure that abuse and neglect concerns are appropriately reported and immediate actions are taken to ensure client safety.
  • Assess the well-being of clients and mitigate any identified risk with the direct supervisor and other appropriate team members.
  • Call and visit biological and adoptive families in their homes to assure the quality and appropriateness of services provided to each client.
  • Provide services to each client based on the service needs defined in the Individual Service Plan (ISP); This includes transportation to appointments as necessary.
  • Participate in the development of an ISP, ITP, CCP to meet program requirements to establish the services to be provided to for each client.
  • Coordinate services to assure effective communications with the treatment team as well as making referrals to community resources for related services.
  • Work in collaboration with the Department of Social Services to assist families in also completing their DSS treatment plan.
  • Utilize various assessment tools to prioritize and target problem behavior for use in the ISP, such as the FAST Assessment.
  • Maintain productivity requirements based upon employment status as defined in the Bair policy.
  • Coach caregivers in the consistent implementation of home-based behavioral supports, if applicable.
