Responsibilities
  • Uphold and further the mission of the YWCA of Hawaii Island, which is dedicated to eliminating racism, empowering women, and promoting peace, justice, freedom, and dignity for all.
  • Provide supportive, home-based services to families in accordance with Healthy Families Program policies and procedures.
  • Contact families to schedule and conduct home visits, ensuring services are culturally sensitive and meet individual family needs.
  • Foster positive, trusting relationships with families, offering emotional support and guidance on parenting, child development, and overall family well-being.
  • Collaborate regularly with a multidisciplinary team, consulting on family needs and developing strategies to address identified risk factors.
  • Communicate observations related to child safety, mental health, domestic violence, and substance abuse to the appropriate team members or agencies.
  • Refer families to relevant community resources and programs, such as DHS or WIC, and support transitions to other services when needed.
  • Participate actively in staff meetings, child team discussions, and Family Goal Plan (FGP) reviews and meetings.
  • Help implement and track progress on Family Goal Plans, including documenting updates and strategies.
  • Conduct approved developmental screens and assessments with guidance from the professional team.
  • Maintain timely, accurate, and confidential records in accordance with program standards, utilizing designated tools and systems.
  • Complete required organizational, contract, and program documentation.
  • Attend all required trainings, meetings, and professional development sessions.
  • Perform other duties as assigned, including occasional evening and weekend work.
