Bay Once Care Services intends to provide several services under the NDIS registration
group that would provide the participants choice of activities they would love to engage
in as well as a high standard of personal service performed by excellent and qualified
employees of the company.
We are seeking a qualified and experienced Family Support Worker to join our team
at Bay One Care Services in North Lakes, QLD. We understand the challenges of at-home care; as such, the pay rate we offer is $80,000 to $85,000 with 11.5%
superannuation on top. We also cover the cost with the km’s travelled between shifts
and any transportation on shift.
We’re looking for those special few who genuinely want to engage with and contribute
to our client’s independence and well-being.

SKILLS, EXPERIENCE AND CERTIFICATES/LICENCES REQUIRED:

• Diploma in Community Services or Health Related Field.
• Minimum 1-year Full Time experience
• Driver’s licence.
• NDIS Workers Screening Card (paid)
• First Aid Certificate
• CPR Certificate
• Australian Criminal History Check

Experience:

KEY RESPONSIBILITIES:

• Identifying and assessing client needs, and designing, implementing, and evaluating educational, training, and support programs
• Conducting interviews to evaluate clients’ challenges and determine the scope of their difficulties
• Monitoring client progress and preparing reports on outcomes and developments
• Referring clients to appropriate agencies and services for further assistance
• Assessing community needs and available resources in areas such as health,welfare, housing, employment, and training
• Engaging with community groups, welfare organizations, government agencies, and private businesses to address community issues and promote awareness of available services
• Providing family support, education, and care for children and people with disabilities in adult service units, group homes, and public institutions
• Supervising individuals on probation or parole to support rehabilitation and reintegration
• Assisting young people in managing social, emotional, and financial challenges
• Preparing funding proposals, resource requests, and reports for submission to government departments and relevant agencies
