While no two days are the same, each day presents an opportunity to make a meaningful impact. This role is inherently flexible and will vary based on the unique needs of the senior living community, the provider team, and the residents we serve. It may include ongoing assignments or one-time tasks, both clinical and non-clinical in nature, and may evolve over time. Our aim is to be as transparent as possible about the types of work involved, so you can make an informed decision about your fit for this position. Below are examples of recent responsibilities carried out by our team in various communities. Please note that this is not a comprehensive list, nor is it presented in order of priority: Participating in and chaperoning community outings Planning, supporting, or leading activities in the activity room Obtaining and recording patient vitals (e.g., weight, blood pressure) and communicating updates Supporting care management efforts, including dementia program support and coordination of care Providing administrative and logistical support to both onsite staff and the broader Care Management team Collecting, organizing, and maintaining patient documentation Assisting with new resident move-ins, performing room checks, and delivering meals to those who are unable to access the dining area Collecting urine or fecal specimen kits for mailing (note: direct patient collection is not performed) Assisting with marketing events—including setup, cleanup, food prep, and guest support At its core, this role is about connection. Our team is instrumental in fostering strong relationships with community staff, residents, families, and our clinical partners. When not attending directly to resident needs, the Memory Connect Wellness Coordinator will support our Care Management team under the direction of their supervisor. The overall success of this role is measured by improved resident and staff satisfaction, increased enrollment in our continuum of care services and GUIDE dementia support program, and the strengthening of our presence and partnership in each community.
