Community Healthcare of Texas is currently recruiting for a Field LVN. $2,500 Sign-on bonus POSITION SUMMARY Assist the RN Case Manager in the plan of care for the patient. ESSENTIAL FUNCTIONS * Assists RN in case management of total plan of care of patient/support persons by coordinating with team members and communicating with IDG in accordance with organizational policies, procedures, and regulations that define the scope of practice. * Complete, maintain, and submit accurate and relevant clinical notes regarding the patient’s condition and care given.  Record pain/symptom management changes/outcomes as appropriate. * Administer medications and treatments as prescribed by the physician. * Document nursing care and significant interactions with patients, support persons, and team members. * Assess patient/family response to care and participate in follow-up with bereaved families with various types of contact. * Manage assigned patients equally, regardless of severity, contagion, or complexity of diagnosis. * Participate in on-call rotation POSITION QUALIFICATIONS * Licensed Vocational Nurse licensed by the State of Texas, required  * Hold a valid driver’s license and have a dependable means of transportation * 1 year of nursing, hospice, or oncology experience preferred * 1 year LVN experience in an acute care setting required. * Proficient skills using a computer are required; Microsoft Office suite experience preferred BENEFITS * Competitive Pay * Generous Paid Time Off Programs * Company-provided Life Insurance, Short- and Long-Term Disability  * Medical, Dental, Vision  * Flexible Spending Account and Health Savings Account  * Employee Assistance Program * Retirement Savings Plan  * Mileage reimbursement for work-related travel Reasonable Accommodations Statement  To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
