We are currently hiring for a full-time Field RN in our Buffalo, New York Area! As a Field RN at Venture Forthe, you will be responsible for the medical oversight of clients in their homes. Venture Forthe is seeking a compassionate and skilled Field Registered Nurse to support clients throughout the community while mentoring and supervising our Personal Care Aides (PCAs). If you’re an RN who loves building relationships, teaching others, and making a direct impact on clients’ lives, this is the perfect role for you. ***$10,000 Sign on Bonus for Full-Time***RN hires will receive bonus payments, equally divided on a quarterly basis upon full-time work having been completed as follows: After 90 days of continuous working. After six months of continuously working. After 270 days After one year of employment As a Field RN, you will be the clinical backbone of our home care program. Your responsibilities will include: Conducting in-home client visits, assessments, and reassessments Developing and updating individualized Plans of Care Ensuring services match client needs and safety requirements Supervising, provide in-service education, and conduct on-the-job training for PCAs as needed Communicating with physicians and securing necessary medical orders Monitoring client safety and appropriateness for community-based care Supporting interdisciplinary care coordination Completing accurate clinical documentation and participating in quality improvement Participating in the nursing on-call rotation (approximately 10 times/year) Acting as an advocate for both clients and staff This role blends clinical expertise, case management, and direct staff support—making every day varied, meaningful, and rewarding. Ideal candidates will have the following: Active NYS RN License Graduation from an accredited nursing program (Diploma, BSN, or MSN) 1 year of acute care experience preferred 3–5 years of home care experience preferred Strong understanding of RN and paraprofessional scope of practice Excellent communication, critical thinking, and organizational skills Ability to complete accurate clinical documentation Comfort traveling to client homes and community settings Physical & Work Requirements Ability to lift up to 50 lbs occasionally Work includes office time, travel to client homes, and community environments Must be comfortable conducting visits independently
Responsibilities
As a Field RN, you will conduct in-home client visits, assessments, and develop individualized Plans of Care. You will also supervise and provide training for Personal Care Aides while ensuring client safety and care coordination.
