Description Field Team Registered Nurse (RN) Position Summary Oak Orchard Health is seeking a compassionate and community-focused Registered Nurse (RN) to join our Patient Engagement Services (PES) team. The Field Team RN provides direct nursing care and health outreach services to Migrant and Seasonal Farm Worker populations through mobile and community-based healthcare initiatives. Working collaboratively with providers, community health workers, and multidisciplinary team members, the Field Team RN plays a vital role in improving access to healthcare for underserved populations. This position supports Oak Orchard Health's mission by delivering high-quality, patient-centered care while ensuring efficient clinical operations in field settings. This position requires availability on Tuesdays from 3:00 PM – 8:00 PM and Thursdays from 3:00 PM – 8:00 PM, with opportunities to pick up additional hours supporting Patient Enga gement Services or within Oak Orchard Health Primary Care offices. Essential Duties and Responsibilities Patient Care & Clinical Services Perform age-appropriate patient screenings and assessments, documenting and reporting findings as appropriate. Obtain and record patient vital signs accurately and efficiently. Administer vaccines, injections, and PPD placements in accordance with established nursing protocols and provider direction. Ensure accurate documentation of immunizations and treatments, including NYSIIS reporting requirements. Assist in providing patient education and support to promote health, wellness, and disease prevention. Documentation & Care Coordination Prepare and update patient medical records, including immunization histories, medication lists, problem lists, and medical, social, and family histories. Maintain accurate, timely, and compliant documentation within the electronic health record (EHR). Complete required patient and provider paperwork and assist with care coordination activities. Ensure documentation meets Oak Orchard Health standards and regulatory requirements. Mobile Unit & Clinical Operations Maintain, clean, and organize the mobile healthcare unit between patient visits and as needed throughout clinical operations. Monitor and maintain adequate supplies and equipment to support safe and efficient patient care. Perform machine maintenance and point-of-care testing (POCT) quality controls according to manufacturer recommendations and industry standards. Monitor expiration dates of vaccines, medications, injections, instruments, samples, and clinical supplies. Maintain a clean, organized, and professional work environment. Quality & Compliance Adhere to infection prevention and control policies and procedures. Promote patient and staff safety by following all clinical and operational protocols. Ensure compliance with Oak Orchard Health policies, procedures, and quality standards. Participate in team meetings, training activities, and community outreach initiatives as assigned. Perform other duties and responsibilities as requested by management Requirements Qualifications Education & Licensure Graduate of an accredited Registered Nursing program. Current New York State Registered Nurse (RN) license in good standing. Current Basic Life Support (BLS) certification for Healthcare Providers or equivalent. Experience Minimum of 2–3 years of nursing experience in Primary Care, Internal Medicine, Family Medicine, Public Health, Community Health, or a related clinical setting preferred. Experience working with diverse and underserved patient populations preferred. Knowledge, Skills & Abilities Ability to provide compassionate, culturally sensitive care to a diverse patient population. Experience utilizing Electronic Medical Record (EMR/EHR) systems. Strong organizational skills and attention to detail. Excellent verbal and written communication skills. Ability to work independently while collaborating effectively within a multidisciplinary team. Flexibility to work in community-based and mobile healthcare environments.
