The successful post holder will work as part of the Urgent Care Response Service team, undertaking an initial triage assessment and working with individuals to support risk management and safety plans for people in crisis.
The role will also include signposting and referring to appropriate services, and there is an expectation that the post holder will build strong links with 3rd sector and partner agencies to collaboratively support the needs of an individuals in crisis.

THE POST HOLDER WILL:

• Work as part of a large multi-disciplinary team liaising with other teams within the First Response Service, the wider organisation and various third sector organisations.
• Provide a triage and signposting service for patients contacting the crisis line in a self-defined mental health crisis, escalating when appropriate.
• Will engage with people who are in a crisis situation and enable effective decision-making to ensure safety.
• Will assist in the formulation and delivery of safety plans working in a collaborative way.
• Ensure accurate record keeping and participate in audits, data collection and other administrative duties when required.
