Description We are seeking a compassionate, organized, and patient-focused Float Specialty Nurse Case Manager to provide advocacy, care coordination, and guidance for patients with complex medical and social needs at our Dora Street and Lakeview Health Centers. Working collaboratively with multidisciplinary care teams, the Nurse Case Manager ensures patients receive high-quality, comprehensive care while empowering them to make informed healthcare decisions and improve their overall quality of life.
