Description

Basic Purpose
The Foster Care Team Coordinator (FCTC) is responsible for providing supervision, guidance and support for the Foster Care Team Leaders and their field staff, i.e., Foster Care Case Management Staff. This will include: ensuring that Foster Care field staff are performing within the expectation of their roles; ensuring all required documentation is complete and compliant with regulatory standards; coordinating intakes and admissions; overseeing staff scheduling; and conducting and documenting performance evaluations.
The responsibilities of the Foster Care Team Coordinator include but are not limited to the following:
Duties

  1. Basic Responsibilities
  • Support Foster Care Team Leaders and their field staff in their role and provide guidance and assistance related to policies and procedures as necessary.
  • Assist in reviewing all relevant documentation completed by Foster Care Case Managers and Foster Home Coordinators that is both internally and externally regulated for compliance and provide feedback on areas needing improvement on an ongoing basis.
  • Assist in coordinating client intakes and admissions and assist Team Leaders in assigning caseloads to Case Managers.
  • Set schedules for staff to ensure appropriate coverage is provided for all cases, to include: court appointments; home visits; utilization reviews; family contact; and internal and external meetings.
  • Work collaboratively with the clinical team to maintain the highest quality of care for all clients in the Foster Care Program.
  • Provide professional development opportunities and conduct performance evaluations for Team Leaders and assist Team Leaders in creating performance evaluations for field staff.
  • Provide training for Foster Care Case Managers and Foster Home Coordinators and ensure learning opportunities are beneficial and therapeutically appropriate in the areas of growth, best practices, compliance, and regulation.
  • Attend all required clinical staff meetings to ensure the highest level of communication is maintained within the clinical team.
  1. Review Compliance with Clinical Care and Programs
  • Supervise, complete, and verify that home and caseload utilization is effective for the client census and appropriate for clients.
  • Ensure positive communication and support occur between the clinical team, foster care field staff, foster families and biological families. Make referrals to appropriate members of staff, community providers, and subcontractors when specialized services are needed.
  • Work with Team Leaders to verify that Child Family Team Meetings are conducted within specified and required timeframes.
  • Provide or delegate training to Case Managers on case management practices and procedures regularly and as updates to regulations occur.
  • Help Foster Care teams maintain focus on the progress toward permanency for each client.
  • Monitor caseloads of Case Managers, which is not to exceed 10 therapeutic clients.
  • Work with Foster Home Coordinators and the Director of Foster Care on exploring new clients and making appropriate placements.
  1. Administrative
  • Maintain all requirements for Foster Care field staff according to policy and procedures throughout the continuum of services under the direction of the Director of Foster Care with the assistance of the Team Leaders, Foster Care Compliance Coordinator, Administrative Assistant, and Human Resources.
  • Hold Team Leaders accountable for documentation including the use of corrective action.
  • Participate in ongoing meetings as required with Foster Care field staff to ensure concerns, compliance, and grievances are addressed.
  • Document Supervisory meetings with Staff and record any concerns and progress.
  • Complete 90-day and Annual Evaluations of Foster Care Team Leaders and ensure they are complying with Foster Care Field Staff.
  • Inform the Director of Foster Care of staff and program concerns.
  • Responsible for contacting the Director of Foster Care and/or CEO of emergency decisions made in direct supervisor’s absence.
  • Assist Foster Care Compliance Coordinator in ensuring licensing standards and contract requirements are met by Case Managers and Foster Home Coordinators.
  • Perform other duties as assigned.

QUALIFICATIONS

  • Masters degree in Social Work, Psychology, Counseling, or other closely related field.
  • Minimum of five (5) years of experience in Foster Care.
  • Minimum of three (3) years of supervision.
  • Excellent writing skills.
  • Excellent computer skills to include the most updated version of Microsoft Office.
  • Excellent phone and communication skills.
  • Positive self-motivator.
  • Adaptable to change.
  • Ability to take direction.
  • Ability to work effectively as a member of the Treatment Team of SSYTS.
  • Possesses and practices good public speaking abilities.

Personnel Practices

  • Working Hours – As needed at 80+ hrs per pay period.
  • Time Off – Saturday and Sunday except when on-call or needed for events

Job Type: Full-time
Pay: $58,000.00 - $63,000.00 per year
