Location: FLC Beacon Hill 1625 South Columbian Way Seattle WA 98108 Full Life Care is a not-for-profit organization, incorporated in 1980. From the beginning, we have been dedicated to improving the quality of life for adults and their caregivers. We help adults of all ages with chronic illnesses and physical or developmental disabilities regardless of their income. We have an amazing opportunity for a Full Life Care - Multi Site Service Coordinator to join our team! The hourly rate for this role is $28.41 per hour. Our Service Coordinator (Multiple Sites): Responds to referrals ffom Seattle Housing Authority (SHA) property management and other sources to provide services to elderly and non-elderly disabled residents. Conducts eligibility screenings and reauthorization screenings for Foundational Community Supports (FCS) services for all eligible residents. Performs regular outreach to individuals and families residing in SHA’s Scattered Sites public housing across King County. Assesses residents’ current service connections and assist them in reconnecting or enrolling with social service providers to meet their needs. Provides direct support and advocacy to low-income residents, including translation services and assistance accessing essential resources such as rental assistance to stabilize housing. Monitors resident progress to evaluate the effectiveness of referrals and stabilization services. Delivers high-quality services by following established standards, policies, and program procedures. Maintains a safe and healthy environment by adhering to infection control protocols, addressing safety concerns, and complying with emergency procedures and training requirements. Establish a respectful and compassionate environment through professional conduct, effective communication, and maintaining client confidentiality. Maintains timely and accurate documentation in compliance with WAC, HUD, and Full Life Care standards. Participates in weekly team meetings and regular case consultations to support coordinated care. Completes assigned projects and tasks as directed by the Program Manager. Promotes the Mission, Vision and Values of Transforming Age in all areas of responsibility, and demonstrate commitment to Culture of Excellence and strategic goals. Performs duties as assigned. What you need to succeed: Bachelor’s Degree in related fields i.e. Psychology, Sociology or Mental Health studies or the relevant combination of education and experience. Driver's License and reliable transportation to travel to client homes in and around King County, WA Experience with social service resources in King County. Understanding of housing systems, social services, and healthcare navigation. Strong organizational and multitasking skills. Effective communication and documentation skills. Proficiency within EHR database systems. CPR certification and HIV/AIDS information training or ability to obtain within 90 days of hire.
