Building rapport is significant in working with the staff. • Ability to communicate effectively with CLNM staff and any outside entities. • Highly effective organizational skills • Open to feedback from multiple sources • Knowledge of community resources for referral purposes (outpatient counseling, physical therapy, medical equipment companies, etc.) • Ability to collaborate with staff and any outside entity. • Ability to manage time effectively while coordinating care for several clients at a time • Empathic, compassionate, and friendly disposition with both clients and staff • Ability to work together with the Director and Clinical Supervisors of CLNM to facilitate communication and information. • Ability to work together with Outpatient MHR Staff in a collaborative and integrated way. • Deal with complaints from all parties and address them as needed or refer to the proper party (MHR/CLNM organizational chart) • Work with the QI Director to develop and improve quality measures and improve the performance of the program. • Ability to run reports and disseminate them to the steering committee. • Provide supervision of an assigned team of care coordinators. • Ability to maneuver well through Electronic health records. • Must be proficient in Word programs, excel, typing • Ability to prioritize multiple tasks at one time. • Documents coordinated activities through BHSD Star and EHR. • Utilizes community resources (food vouchers, housing, etc.) to help the members improve their day-to-day living and extend their quality of life. • Identifies risk factors. Looking into the member's past and present issues concerning both behavioral and physical issues. • Maintain HIPPA and client confidentiality at all times. • Regular and consistent attendance for the assigned work schedule is essential.
