Description Responsible for managing the needs of clients and providing guidance, empathy, education, and support. Assists with helping breakdown the complexities of dealing with insurance companies, doctors, and other health professionals Requirements Full Time, 40 hours, non-exempt Starting pay: 18.27/ hr A bachelor's degree in any human service field is required. Bilingual/Spanish-speaking individuals are strongly encouraged to apply. Building rapport is significant in working with the member. • Educates family members about the client’s physical and behavioral needs. Works closely with the family to understand, accept, and follow medical recommendations. • Administers and completes the initial Comprehensive Needs Assessment (CNA). Identifies the needs of the member through this assessment. • Documents coordinated activities through BHSD Star and EHR. Coordinated activities include, but are not limited to, making doctor appointments, transportation needs, or helping fill out the paperwork with the member. • Promotes the principles of recovery and resiliency. • Collaborates with health professionals to make sure the member gets the proper care. This can include making appointments and attending appointments. • Organizes and creates care plans to help the member achieve their physical and behavioral goals. • Utilizes community resources (food vouchers, housing, etc.) to help the members improve their day-to-day living and extend their quality of life. • Identifies risk factors. Looking into the member's past and present issues concerning both behavioral and physical issues. • Maintain HIPPA and client confidentiality at all times. • Regular and consistent attendance for the assigned work schedule is essential. This position requires a bachelor's degree in any human service field. This position may be home-based at times. The home office needs to meet security measures and the work environment must be free of background noise distractions. Own internet access is a must.
