Independent Living Skills - Community Support

  • Support with daily living needs (i.e. groceries, medication)
  • Applying and securing ID
  • Securing bank accounts
  • Funds disbursement, including financial mentoring and modules
  • Transportation to appointments/ meetings
  • Support to connect youth with a family doctor, optometrist, and dentist available to them as adults, make appointments as required
  • Ongoing awareness and promotion of various community-based organizations/ government services
  • Facilitate/promote access to community-based programs including, cultural, recreation, training based on interes

Life Skills Programming

  • Teaching and mentoring life skills programming, utilize curriculum and hands on lessons and/or integrated into daily living
  • Assistance and teaching skills such budgeting, meal planning, grocery shopping and meal preparation
  • Guidance on hygiene
  • Guidance on healthy relationships, personal safety and wellness
  • Support to address any barriers or challenges youth is experiencin

Responsibilities

Independent Living Skills - Community Support

  • Support with daily living needs (i.e. groceries, medication)
  • Applying and securing ID

Tenancy Management

Supporting Safe Housing

  • regular check-ins
  • teaching and mentoring home safety and tenancy management
  • fostering good relationships with property managers and landlords
  • support in apartment cleaning and restoration
  • support finding apartment at time of transition

Life Skills Programming

  • Teaching and mentoring life skills programming, utilize curriculum and hands on lessons and/or integrated into daily living
  • Assistance and teaching skills such budgeting, meal planning, grocery shopping and meal preparation
  • Guidance on hygiene
  • Guidance on healthy relationships, personal safety and wellness
  • Support to address any barriers or challenges youth is experiencing

Documentation and information sharing

  • Maintain written logs, document pertinent information as it relates to youth activity and programs provided; communicate with team and other supports, adheres to duty to report on significant incidents and/or disclosures.

Evaluation and statistics

  • Supports the collection of statistics and reporting on outcomes, and all evaluation activities.
