Full Time MDS Coordinator Avamere Transitional Care at Sunnyside 4515 Sunnyside Road SE, Salem, Oregon Please apply online at: https://teamavamere.com/ [https://teamavamere.com/] The MDS Coordinator (Minimum Data Set Coordinator) is the primary point of contact for the review and coordination of patient care plans to promote quality care and services to residents. This role works with other nursing department colleagues to ensure that patient care is coordinated and communicated effectively with the clinical team. MDS job coordinators determine the health status, care plans, and procedures for intake of residents according to state and federal standards. The successful candidate will possess a strong understanding of healthcare regulations and have experience in clinical assessment and documentation. * Review resident care plans for appropriate goals, problems, approaches, and revisions based on nursing needs. * Ensure all personnel involved in providing care to the residents are aware of care * Schedule and facilitate care plan * Monitor and guide the completion of PPS and OBRA * Ensure accuracy of clinical assessments and documentation. * Ensure legal compliance with all state and federal guidelines, including, but not limited to following MDS processes. * Manage the utilization review (UR) * Oversee the MAR’s treatment, flow sheets, and physician’s * Chart nurses’ notes in an informative and descriptive manner that reflects the care provided to the resident and resident’s response to care. * Coordinate with other departments to ensure consistency in resident care. * Participate in facility surveys by authorized government * Report findings and make recommendations to management. * Act in a supporting role to the Administrator and Director of Nursing of the facility. * Nursing degree from an accredited college or * In accordance with state and federal regulatory requirements, maintain a current, unencumbered, active license to practice as a licensed nurse in this state. * Oregon: RN is required * Washington: RN is preferred * MDS certification * Knowledge and experience in the RAI process, CAAs, RUG categories, and MDS 0. * Knowledge of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care communities. * Experience with electronic medical records and computer documentation Avamere Living is an Equal Opportunity Employer and participates in E-Verify. #clinical95
